It has always been that border towns in both the US and Canada would share various community services, since they were close geographically. And to access the same services in their own countries might take them hundreds of miles farther.
So it makes perfect sense that Obama's mother may have chosen a home for unwed mothers/birth services on the Canadian side. It's not as if she could have foreseen that he would want to be American President decades later!
